Client/Teammate Application and Information

Thank you for your interest in becoming a teammate at CPPC!  Our application process is designed to allow prospective teammates, parents/guardians, and/or family members the opportunity to see our program in action, meet our staff, and discuss the goals/opportunities for each teammate individually.  Below are the steps required to determine participation in our program.

  1. Call and schedule a tour of CPPC at 972-773-9937.
  2. Complete the application and submit application fee.
  3. Interview with prospective teammate, guardians, parents, and CPPC Admissions team.

In addition to the completion of the application, please also submit the following:

  1. Guardianship paperwork (if applicable).
  2. Doctors orders for medication/treatment (if applicable).
  3. Most recent copy of the following:
    1. Full Individual Evaluation (FIE)
    2. Individual Education Plan (IEP)
    3. Behavior Modification Plan (if applicable)
    4. Any evaluations completed by Metrocare, DARS, and/or your provider.

Private pay client fees are as follows:

  • Level I $30/day
  • Level II $35/day
  • Level III $45/day

Level of need will be based on multiple factors and will be decided by the Executive Director upon review of client application.

For clients with HCS, CLASS, or TxHmL benefits, these rates will be discussed with the Provider.

CPPC requires that all payments be made in full at the beginning of the month. Non-payment may result in removal from the program.
