Available Positions:

CONNECTING POINT OF PARK CITIES – PROGRAM MANAGER

The Program Manager (PM) is responsible for helping Teammates achieve their social, recreational, activities of daily living and pre-vocational goals. The PM oversees designing, implementing, and leading all Teammate activities. The PM supervises the direct care staff. The PM assists with administrative and reporting responsibilities. Reporting to the executive director, the PM serves as the ‘acting director’ in the absence of the executive director.

LEADERSHIP

  • Supervises all aspects of service delivery for Teammates.
  • Holds support staff accountable for exceptional service delivery.
  • Maintains and promotes a culture of respect, positivity, and fun!
  • Creates and implements curriculum consistent with ISS requirements to meet Teammate: community, pre-vocational, social, activities of daily living (ADL), and recreational needs.

STAFF and VOLUNTEER MANAGEMENT        

  • Provides leadership and direction to assist program staff in accomplishing work assignments and fulfilling their job description.
  • Oversees and reports on a training program that meets THHSC requirements as well as continuous learning.
  • Provides training, leadership, and direction to program volunteers.
  • Plans, tracks, and documents program staff training.
  • Schedules program staff and community volunteers to deliver excellence while adhering to a labor budget.
  • Creates and trains staff on program data collection and reporting.
  • Conducts program staff performance reviews.

CURRICULUM CREATION and DELIVERY

  • Leads program development and curriculum expansion.
  • Identifies the need and develops activities allowing Teammates to participate to the maximum of their individual abilities.
  • Ensures the Program exceeds THHSC licensing requirements.
  • Assigns staff to a specific program and is the lead in ensuring program activity and CPPC is operating at the highest and most efficient level.

DATA COLLECTION and REPORTING

  • Responsible for the accurate, timely reporting of program activity.
  • Ensures the entry of progress notes, and incident reports into the CPPC database.
  • Aggregates individual Teammate performance to report on overall program impact.

COMMUNICATION

  • Creates, maintains, and communicates annual program operations calendar.
  • Communicates with Teammates and their families/guardians on all matters related to Teammate well-being and program execution. Ensures the executive director is informed on the same and cc’d on written communication.

GENERAL

  • Assists Teammates with personal care needs.
  • Drives Teammates in CPPC van or bus.
  • Provides valid proof of insurance for personal vehicles.
  • Follows CPPC cell phone policy.
  • Effectively executes all other duties as assigned.

Education and/or Experience: 

A licensed Occupational, Recreational, Vocational, or Behavioral Therapist, or a Bachelor’s degree in Human Services or Special Education – Transition Specialist. A Master’s degree is a plus. 5 years or more of experience in providing care to adults with Intellectual and Developmental disabilities.

Must have a minimum of 3 years and preferably 5 years of experience managing diverse and high-performing teams.    

Physical Demands:

Must have good health to maintain an acceptable attendance record. Must be able to assist with lifting objects or individuals (a minimum of 50lbs for one person lift and 150lbs for a two-person lift) utilizing approved techniques. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Program Assistant

Program Assistant: The Program Assistant is a member of the direct care staff, responsible for all aspects of care for adults with intellectual and developmental disabilities. Part-time and on-call positions are currently open. Qualifications: Must possess a passionate desire to serve adults with intellectual and developmental disabilities. High school diploma or GED. $16.00/hr. plus depending on experience.
